Research Ethics Committee (REC) Requirements

For research proposals that had been reviewed and accepted by a technical panel (such as the Research Review Committee in the Research Chair Program), the proponents are expected to apply for ethics review to the ADZU-Research Ethics Committee. The proponents are to submit these requirements.

  1. Letter of Intent addressed to the REC Chairperson
  2. Protocol (Research Proposal)
  3. Informed Consent Form
  4. Technical Review Clearance
  5. Curriculum Vitae
  6. Form 011 Protocol Evaluation Worksheet
  7. Form 012 Informed Consent Evaluation Worksheet
  8. Form 013 Applications for Research Ethics Review

 

These requirements are to be emailed to adzurec@adzu.edu.ph. 

Additional Instructions For Students at the Graduate and School of Medicine

  • Submit 2 printed copies of the 8 requirements above to room 205, in the Fr. Ernesto Carretero Building, Salvador Campus, Ateneo de Zamboanga University, La Purisima Street, Zamboanga City.
  • Pay your ethics review fee. There are two ways to pay:
  1. Visit at the Finance Office; then present the official receipt to room 205. 
  2. For ONLINE PAYMENTS, send a Screenshot of the Proof of Payment and email this to the adzurec@adzu.edu.ph

Research Ethics review is completed within two (2) to three (3) weeks from date of submission.

Progress notes are to be submitted if the research is not completed within a year the research ethics clearance was granted. Upon completion of the study / research, a final soft (electronic) and a hard copy is to be submitted.
